Office Hours

Our office hours are Monday through Friday from

 

 

Appointments

Appointments with any of our surgeons can be scheduled by calling our main office. Whether you are a new or returning patient, please bring your insurance card with you at the time of your visit. If you are a returning patient and your insurance policy has changed, please notify our office prior to your visit so that we can verify our participation in your health plan.

New patients can expedite the waiting time in our office by printing and completing the Patient Information & Medical History forms (available on this page) prior to the visit. Patients taking multiple prescriptions should bring a list of the medications they take and the dosage.

Health Insurance & Billing

We will verify insurance benefits for our patients prior to their appointment. Copays, deductibles, and co-insurance fees are collected at the time of service. Our office accepts cash, personal checks, money orders, Visa, MasterCard, American Express, and Discover.

 

Disability Forms/FMLA Forms

If you have a disability or FMLA form that needs to be completed by our office, please allow five business days for processing. There is a $15 fee for each form. The forms can be mailed, faxed, or picked up upon receipt of payment.

Medical Record Requests

Medical record requests must be made in writing and authorized by the patient. All medical record requests will be released within 15 business days of the written request. There may be a charge for the release of medical records.

 

Prescription Refills

Please call your pharmacy for any refill requests. The pharmacy will contact our office with any questions regarding your request. Refill requests will not be processed after regular office hours or over a weekend.

Patients under the age of 18 must be accompanied by a parent or legal guardian.
